The DIEP flap breast reconstruction

DIEP flap breast reconstruction is the most widely used microsurgical technique for breast reconstruction. The reconstructed breast consists of skin and abdominal fat, resulting in a very natural appearance with consistent skin texture and long-term stability.

The procedure involves abdominoplasty, which includes the removal of excess skin and fat from the abdomen. In cases where there is insufficient abdominal fat, tissue can be harvested from the thighs, specifically below the gluteal region (PAP or Posterior Artery Perforator flap).

This technique of breast reconstruction after mastectomy involves harvesting skin and fat from the abdomen along with their vascularized vessels and transferring them to the reconstructed breast area. The vessels are then microscopically connected to vessels in the chest or under the arm.

This breast reconstruction surgery represents one of the latest advancements in microsurgery, as it allows the abdominal wall to remain intact. Unlike the TRAM flap technique, which requires harvesting abdominal muscle along with tissue, the DIEP flap technique is increasingly favored for breast reconstruction.

Regarding the removal of abdominal skin and fat, it involves a true abdominoplasty that offers similar benefits to cosmetic abdominal surgery. In particular, the final scars from the tissue removal will be nearly identical to those of an abdominoplasty, typically very low on the pubic area (concealable under clothing) and possibly around the belly button.

The patient arrives at the clinic in the morning. This breast reconstruction surgery is performed under general anesthesia in the operating room. It is typically carried out by a dual surgical team, often involving another surgeon. Hospitalization usually lasts between 4 and 7 days.

It is necessary to wear a compression garment for one month following the DIEP flap breast reconstruction surgery. Anticoagulant treatment will be administered for 10 days to prevent blood clots.

The cost of DIEP flap breast reconstruction varies between 5000 and 10,000 euros. This breast reconstruction procedure is covered by health insurance.
