For over 20 years, Botox treatment has been used to manage axillary hyperhidrosis (underarm sweating). In rarer cases, it may also be recommended to alleviate palmar-plantar hyperhidrosis (sweating of the hands and feet).

Sweating is a normal response to body temperature increase (heat, physical activity, fever) and emotions. Sweat is produced by eccrine and apocrine glands beneath the skin, regulated by the sympathetic nervous system and controlled by the hypothalamus. Persistent or transient dysregulations can lead to excessive sweating, causing discomfort and self-consciousness.

The treatment of hyperhidrosis with botulinum toxin injections takes place in the doctor’s office during a consultation with the surgeon. The examination allows the practitioner to assess the treatment indication.

The injections are nearly painless due to the use of extremely fine needles. Patients may optionally apply a topical local anesthetic (such as EMLA cream) beforehand. Immediately after the procedure, the individual can resume their daily activities.

It is recommended to avoid taking anticoagulants, anti-inflammatories, or certain antibiotics for about ten days prior to the consultation. The toxin is also not advised for pregnant or breastfeeding women. Additionally, high-level athletes should disclose their status, as injections may cause minimal muscle tone loss.

Botulinum toxin treatment for hyperhidrosis begins to take effect immediately. However, noticeable effects typically appear after two to three days. The optimal effectiveness of the treatment is achieved within the first month following the injection.

The duration of action varies depending on each patient, the concentration of the toxin, and the total dose administered. Generally, the treatment remains effective for approximately 6 months.

Beyond this period, sweating returns to its natural rhythm. To maintain the benefits, patients should undergo a repeat treatment after this 6-month period.
